Home Fall-Risk Checklist

Falls are not an inevitable part of aging. Many fall hazards can be identified and corrected before an accident happens.

Walk through your loved one’s home and check each area carefully.

Floors & Walking Areas

  • Walkways are clear of boxes, cords, shoes, pet items, and clutter

  • Furniture does not block common walking paths

  • Loose or unsecured rugs have been removed or secured

  • Flooring is even and in good condition

  • Frequently used items can be reached without climbing or stretching

  • Electrical cords are kept away from walking areas

Stairs & Steps

  • Stairways are free from objects

  • Steps are even and in good repair

  • Handrails are secure

  • Stairs are well lit

  • Light switches are easily accessible

  • Outdoor steps are kept clear and maintained

Bathroom

Bathrooms deserve special attention because wet and slippery surfaces can increase fall risk.

  • Grab bars are available where appropriate near the shower, tub, and toilet

  • The shower or tub has a nonslip surface

  • The bathroom floor is kept dry and free of loose rugs

  • Frequently used toiletries are easy to reach

  • Adequate lighting is available during the day and at night

  • A shower chair or other appropriate equipment is available if recommended for the person’s needs

Bedroom

  • There is a clear path between the bed and bathroom

  • A lamp or light switch can be reached easily from the bed

  • Nightlights illuminate the path to the bathroom

  • Shoes or slippers fit securely and have appropriate traction

  • Items are not stored on the floor next to the bed

  • A phone or method of calling for help is accessible

Kitchen

  • Frequently used food, dishes, and cookware are within easy reach

  • The person does not need to stand on chairs or stools to reach everyday items

  • Spills are cleaned promptly

  • Walking areas remain uncluttered

  • Lighting is adequate

Lighting

  • Hallways are well lit

  • Entryways are well lit

  • Nightlights are installed where useful

  • Burned-out bulbs are replaced promptly

  • Light switches are easy to reach

Outside the Home

  • Sidewalks and pathways are even and clear

  • Leaves, snow, and ice are removed

  • Outdoor stairs have secure railings

  • Entryways are adequately lit

  • Frequently used entrances are easy to navigate safely

Don’t Forget These Additional Fall Risks

A safe home is only part of fall prevention.

Families should also pay attention to:

  • New dizziness or weakness

  • Changes in balance

  • Difficulty getting up from a chair

  • New falls or near-falls

  • Vision problems

  • Changes in walking

  • Medications that may contribute to dizziness or drowsiness

Medication questions and new medical symptoms should be discussed with the appropriate healthcare professional.

After a Fall

Do not assume that a fall is simply part of getting older.

If your loved one has fallen, has repeated near-falls, or has developed new difficulty walking or transferring, let their healthcare provider know.

A caregiver can provide supervision and assistance within the person’s care plan, but professional home care does not replace medical evaluation when there has been a significant change in health or mobility.
